Could anyone tell me if there is a hierarchy in applying V-Codes. Say for instance if the patient has a strong Family hx of Colon Ca (both mother and father) and the patient has a Personal Hx of Colon Polyp. Thanks for your help.
I always code personal history prior to family history. I feel the patients own history is more important so that is why I put it first. Hope this helps!
