How should "application of skin substitute" be billed on the PB side for Low Cost Skin Substitute. I am getting a denial for C5271 from Medicare. The Claim was paid on the HB side.

C5271 only applies for facility billing. For the physician, bill 15271 for either high or low cost skin substitute on the trunk, arms, or legs, first 25 sq cm.
