The patient was last seen 3 years ago and is returning for another colonoscopy. The indications on the procedure report states High risk colon cancer surveillance; Personal history of polyps. Polyps were found and removed with hot snare. I need some help in regards to coding surveillances. Can both V76.51 & V12.72 be coded or should a follow up code V67 with V12.72 be coded or just the polyp findings. Any information will be helpful.
