We have a patient that has metastatic endometrial cancer with metastatic disease to the lung. She is status post TAH and oophorectomy in 1987. She currently and has been treated previously with Lupron for the endometrial cancer for many years. Should the account be coded as:

V58.11, 197.0, V10.42, V88.01 OR

V58.11, 179, 197.0, V88.01

Yes. we can take the first answer. As per coding quidelines in ICD-9-CM in neoplasm topic (2c) , if the primary malignancy is excised (V88.01) and no evidence of existing cancer in that site means we have to take history codes (v10.42) only.



In your scenario lupron treatment is given for endometrial cancer. Because of there is no existence of that anatomical site (endometrium), lupron treatment may be for prophylactic. I suggest you can take V58.11,197.0,v10.42,v88.01.

As long as you are treating the cancer you code it as active. Since Lipton treats endometrial cancer you code it as active not history of. So I chose second answer
