He is a previous smoker, quitting in 1993. He smoked 1 pack a day for 30 years. He does chew tobacco and has since 1993.
Q: Do you code the current chewing F17.220 w/HX Z87.891 of or just the chewing. Any thoughts as the HX of doesnt state of specific nicotine just HX of nicotine dependance.
Pt is being seen for lung cancer.

No. We can’t code both at one time as per Exclude notes.
All F17 Series codes are indicate the Nicotine dependence. It may be Chewing or Cigarettes or any other tobacco product.
Z87.891 Is personal history of nicotine dependence.

you can't code hx of nicotine dependence when he is still using chewing tobacco, hx of nicotine dependence dictates the use of nicotine regardless of where the person gets his nicotine, this code does not states the hx of cigarette, chewing tobacco, cigar, etc.. but the dependence of the substance itself which is the nicotine.
