"History of" vs "in remission"

Does anyone have information/resources that define how to differentiate a history of alcohol dependence from alcohol dependence in remission? DSM 5 criteria define early remission as no alcohol use criteria met for at least 3 months and sustained remission as no alcohol use criteria being met for > 12 months. Many alcoholic patients are not in active treatment with ongoing therapy. The difference between the codes is significant to HCC coding. Thanks! :cool:
