Looking for opinions and references if possible. This patient is establishing care with Family Medicine and they have chronic conditions which the provider could have documented the status for to obtain HPI credit but did not. I am being advised that the highlighted words should be considered duration, location and quality but I consider this medical history...this is Family Medicine and the provider states Cardiology is treating.
