home health and TMC billing

Does anyone have any idea on how to bill home health and TMC for private insurance?
I had billed 99374 for home heath and 99496 for TMC but got denied by Bx for bundle.

If you read CPT, you'll see that they cannot be billed together in the same month. They are bundled. Pick one or the other, but make sure you meet all of the documentation critiera. That alone may make the decision for you.
