Wiki Home Health Certification -reporting the certification

Can anyone tell me the correct pleace of service for reporting the certification and re-certification for home health services?

We are a family practice. I was told by our Medicare carrier, Trailblazer, to use office (11) as the place of service. I hope this helps!
