Our home health agency is now accepting patients with Medicare Advantage plans. We are experiencing significant rejections for a variety of reasons due to limitations with our EHR. Would anyone be willing to chat or share a sample template of how these claims should be formatted? Rejections include Bill type, HCPCS, codes not valid for Medicare, HIPPS, RAP?NOA, Fips, etc. We handwrote this information in and sent appeals without success.. Primarily these are Fallon and Tufts senior plans. Any guidance would be appreciated.
