I have a patient who was already seen at our office for the first time as a new patient 99204 and pt requested a home visit after. If I bill the new patient for the first visit, how do i bill the first home visit? would the first home visit can be bill as 99349? thanks
 
Correct they are established for the home visit. The full range for established is 99347-99350 so which ever applies based on the documentation could be submitted.
 
if the patient presents for general yearly as a new patient then use the new patient preventive codes . you cannot use an office visit and a preventive visit on the same day if there is no abnormal finding by the provider.

Not just abonomal but a significant separate E&M needed. Minor issues wouldn't be reported with a separate E&M.
