If a provider (MD/NP) is seeing someone in their home (private residence) but it is a community visit per their health plan or a follow-up after hospitalization, would it fall under Home Services E/M (99341-99350) or Preventative Medicine Services (99381-99397)?
 
Sounds like a sick visit

Preventive visits are those for which you have NO chief complaint, but are a regularly scheduled (usually annual) check-up.

Sounds to mee like your visits are NOT preventive, so code the appropriate home-based visit code.
