My MD visited a patient in the Assistant Living Facility and ordered medication for her. Can a MD bill separately for a patient in an Assisting Living Facility who is not affiliated with the facility? If another MD also bills for this same patient maybe for Care Plan oversight? I billed a 99339 with 13 also denied as a non billable code, needs to be bundle according to Medicare, not stated in the 2015 CPT book. Code book says to also see 99374 and vice versa. Do I bill 99339 and 99374 together? If , what codes do I bundle together?
