Has anyone had any luck billing compounded hormone pellets for hormone replacement therapy? We use compounded testosterone and estradiol hormone pellets of varying dosages through Asteria Health. I'm billing code J7999 with the description of the drug, manufacturer, and dosage, but it keeps getting rejected by BCBS for NDC and procedure code. The NDC code is what's listed on the pellet package. Any idea what I'm doing wrong? The rejection reasons are below. Thanks in advance!

Acknowledgement/Rejected for Invalid Information-The claim/encounter has invalid information as specified in the Status details and has been rejected - NDC number
Acknowledgement/Rejected for relational field in error - Procedure code for services rendered
