Hello!! I'm looking for some advice.
Dr documented that pt is coming to have hormonal replacement therapy, E/M code was billed w/ (Z79.890) and the claim came back as a wrong dx, also menopausal symptoms were not mentioned at all in the chart. How this should be documented correctly??? Thanks in advance!!

Hi there! I would assign N95.9 (Menopausal and other perimenopausal disorders), or Z78.0 (Menopausal State) if your progress note supports either diagnosis.
