Wiki Hospice Modifiers and Initial vs Subsequent fx

My team admitted a patient, cared for several days, the patient was transferred to hospice where one of our PA or NP follows the patient as well.
When patient was originally under our service she had two fractures, obviously initial encounter. Her hospice diagnosis is for NASH.
If we're following the patient in hospice do we
a) continue coding as initial encounter for fractures-- or would this be a subsequent at this point
and b) would we use a GV or GW modifier- I think I'm confusing myself on when to use either one. If she was treated for fractures before would we still use GV, or would we use GW since she was admitted to hospice for NASH not the fractures?
