How do I bill Medicare for a Hospital admit by a Family Doc then hours later a DIFFERENT Family Doc transfered/discharged same patient on the same day?

Same practice?

If the two physicians are in the same practice and the same specialty, they are considered the same doctor. You would use the 99234-99236 code for admission/discharge on the same DOS.

You should have some internal policy regarding whose name/NPI this service is billed under (we use the admitting doc).
