When a PA sees a patient in consult at the hospital for the attending doctor who is on call - the top of the consult says attending DR. X and on the bottom of the transcription says PA dictated for DR. X - Is it appropriate to bill under the attending DR. X or should it be billed under the PA? we have a disagreement in our office about this.

If this was the PA seeing the patient then it must be billed under the PA NPI. You cannot bill a consult as a shared encounter so it would not matter if both providers saw the patient. If both did see the patient and both wroth separate notes then you would pick one note and bill the provider that wrote that note. Incident to does not apply in the facility setting.
