Can anyone tell me if there is any written rules regarding how notes are to be written when a Nurse Practioner or Physician Assistant sees a patient in the hospital and the doctor also does an exam and comments that the patient was seen and examined by me, reviewed the PA's notes and included my documentation (attestation) We are told by an auditor that the doctor can not comment on the same page as the PA it must be a separate note. I understand that as of 1/1/12 the attestation must be done but I am looking for something in writing regarding this.

I would like to know what you find out. We have physicians documenting on the residents notes, and adding their attestation statement at the bottom. Let me know what you find out please.
