When coding an inpatient initial visit, the history & exam requirements call for a detailed or comprehensive component but frequently I find the documentation only supports an expanded focus for one or both. How should I code these visits?
 
Palmetto GBA, the Medicare Part B carrier in the state of Ohio has said that in cases such as that, the visit should be coded with the subsequent visit codes (99231 - 99233), which the documentation supports
