We currently have a Locum's provider covering the hospital daily rounds for our family practice clinic. He seems to be a bit old school and I am struggling to code his daily notes. My concern is that what he is charting is not even billable. Here is an example of a daily chart note from the locum provider of a patient who is in the hospital as a swingbed admission (99307-99309).

O breathing comfortably asleep 17 110/60 hr 67
Looks great in his track suit and street shoes, well groomed articulate and full of good stories of his youth

For those more experienced coders, is this even billable at the lowest level? Thank you!!!
