When a hospital bills for outpatient services (example labs) and those services are ordered by a nurse practitioner..... do you use the npi of the nurse practitioner or their supervisoring physician on the claim?

which field on the claim are you filling in? Field 77 is for attending physician, which we never used for outpatient claims, only inpatient.
field 76
