We have a patient that was admitted as observation status on 12/29/07 at
11:30pm and was discharged on 12/30/07 at 11:15am. The pediatrician did not see the patient on 12/29/07, but he was contacted by the hospital ER and was admitted by our pediatrician for observation. The pediatrician saw the patient on 12/29/07 dictated an H&P/discharge. The patient has commercial insurance with Medicaid secondary. How should this be coded?

99219 (initial observation) only
99217 (discharge observation) only or
99234(admit discharge same date) with time listed to show less than 12hrs

Any help would be appreciated.

Thanks for your reply, but it was just brought to my attention that I put the wrong date that our peditrician saw the patient. I should have been 12/30/07. Would that make any difference?
