My question is this. A physician has admitted a patient to Hospital A at an earlier date and then decides to transfer the patient to Hospital B. On the day of the transfer the physician does a discharge from Hospital A (99238-9) and a new admission to Hospital B (99221-3). Both hospitals are acute care facilities.

For the day of the transfer which code(s) can/should be assigned?
 
Subsequent care is the only thing I would feel comfortable with. He didn't really discharge or readmit, he just sent them to another facility for continued care.
