excision with intermediate layered closure ,excision of 7 Sebaceous cyst of the scalp , size between 2cm and 3.2 cm , how do I bill this case?

Total the sizes of the cysts, and choose the appropriate benign excision code.
I disagree.
The guideline for repairs in the same area is to add together, but lesions should be coded separately. I feel like that is a nuance that a lot of people miss. I know for far too long, I personally confused this.
Simple repairs are always included in the lesion removal. Intermediate and complex repairs are usually billable separately.
If you had 3 excisions (remember size of the excision, not just size of the cyst) that were .4cm, 2 were .7cm and 2 were .8cm,
11420 x 3
11421 x 4
If all closures were intermediate, you would add those sizes to come up with 4.2cm total length (3x.4 + 2x.7 + 2x.8
