We need help coding this. The patient is 31 weeks pregnant and presented to an urgent care facility for a bug bite. The doctor states that she's pregnant and her weeks of gestation. He does not state that it is incidental to the pregnancy. Can we use code V22.2? This does not seem to qualify as a complication for a pregnancy. Please help! :)

You cannot use V22.2 unless specifically documented. You can use a 648 code for other current condition complicating pregnancy, followed by the code for the condition .

648.90 seems to fit the criteria. However the bug bite is not listed in the conditions classifiable to this code. 910.4 and 682.0 don't meet the criteria. Hence our dilemma.

Incidental

The visit itself is not for the pregnancy; she just happens to be pregnant. This would qualify for V22.2. The reason for the visit, the first listed dx code, should be the bug bite. UNLESS the provider documents that the bug bite is complicating the pregnancy. In that case, the complication the bug bite caused would lead you to the correct complication code. But this does not sound like the case.
