Is it really necessary to report more than four ICD 10 codes per claim?
 
You may list a maximum of 12. There are some instances such as trauma where you do need more than 4. Also pregnancy complications and sometimes diabetic complications. I regularly use 5 to 6 diagnosis codes for the ER.
