if a patient comes in within 2 days for an ICD lead explant, ICD re implant, and a repositioning of another lead... do all three received the 'unplanned return' 78, or just the one with the highest RVU

Each charge that is within the global of the first procedure will need a modifier, not just the highest RVU. Requiring a modifier on each charge allows you to identify any procedure within global that could be for a different reason, such as staged (58) or unrelated (79).
