Wiki How often can a specialist bill in a SNF?

I am a biller/coder at a nephrologistx office. We just recently started seeing patients in a SNF facility and can not find the guidelines on how many times a patient can be seen in a SNF. Is there a limit on how many visits we can bill for each month? year? any help is appreciated.
