Patient is present stating light bill will be shut off. patient has a chronic condition so patient light will not be shut off. can we bill insurance ?

I see on chart patient is present with forms. Vitals were done HEENT, Neck, Cardiac, Lungs, Psy were all examined no ROS.

How is this even billable?
