Backstory: Patient bought glasses through our optical store. They have their own NPI for their store. Our clinic has their own NPI. Patient has BCBS MN Medicare Advantage plan. We have attempted to send this claim to DMERC B with optical NPI and 12 place of service and it was denied for CO-109. We then sent it to DMERC D with the Optical NPI and 12 place of service and it was denied with CO-109. We are on the North Dakota/Minnesota border so services rendered are in North Dakota.

Then attempted to send it to BCBSMN with an 11 place of service and clinic NPI. Denied for CO-109.

Charges being sent are:

V2020
V2303-RT
V2303-LT
