Help Please ....I am billing for a breast biopsy 19120, and our surgeon did an US localization with markings only, no wires, and also notes interpretation of surgical specimen radiograph. Our surgeon is certified in Breast Ultrasound valid through Oct 2017. My question is, may I bill for a 19120 with a 76642 (to locate mass and put markings on skin only) and 76098? Also, if I can bill all these codes, what needs to be reported in the op note for medical necessity. Are separate reports needed? thanks
