We use an EMR for billing and the insurance companies are requiring all addressed diagnosis be reported on the 1500 claim. The problem is our EMR only allows 4 icd10 codes per CPT code. If patient is only in for one E/M-we are in a family practice office-what to do? Can anyone help me with this? Do you use "dummy" CPT codes? Thank you!
 
You may list only 11 total Dx codes on a claim, you may link any combination of those to a maximum of 4 per line item. The rest of the listed dx codes do not get linked. Link the most pertinent dx codes up to 4 to a single CPT/HCPC II code.
