Wiki How to code 45341 and 45380

I am wondering if there is a way to bill a Lower EUS (45341) and Colonoscopy (45380). The doctor performed both procedures the same day. Per AAPC's National Correct Coding Initiative (NCCI or CCI) edits these codes cannot be billed together in any circumstances. So how would I bill this? Can I bill the colonoscopy w/EUS and biopsy? Or only 45380 because it is the most extensive procedure? (Insurance is BCBS HMO):confused:
 
One of my doctors does both of these procedures during the same session. We bill only 45380. I am keeping my fingers crossed that the changes to the lower GI codes that are supposed to come in CPT 2015 will account for this similar to 43237 vs 43259 or 43238 vs 43242 in the Upper GI code selection.
 
I have a provider billing 45380 and 45341 as well. I was wondering about using code 45391 (because of the use of Ultrasound Endoscoscopic exam) along with 45380 (CCI-approved). Thoughts?
