Hello,
I am in school for medical coding and billing. A case study I am doing is when a patient with history of malignant neoplasm, low anterior resection and colonic pouch for closure of loop anatomy. During the procedure, colitis in the lower intestine was found as well as "an abnormality in the rectum." There was poor prep. I am having a very hard time finding all the codes to use as well as the order in which to use them. So far I have:
Z08
Z85.048
Z90.49
K50.1

Do I use also a code for the poor prep? Do I code the findings, which were not given a diagnosis, just a description, at all? I read in the guidelines that Z08 cames first, then the Z85.048, but then does the code for the previously removed section of intestine? I just wish there was one clear section talking about which codes are coded first, second, etc.

If there is already a forum for beginners like me, I apologize for not posting there. Any help is appreciated! Thank you.

Hi Nephele,
Here are some tips never use Z dx codes first unless it states it in ICD10 manual (see dx block Z00 or Z01or Z51 as first dx) or sometimes for doc checking or encounter for review (ie Z12.11 for colon fecal fit test or birthing a baby dx Z37). Use colitis as first dx K52 and see dx block R93.3 for abnormalities in rectum since part of digestive system. Anytime it says abnormailties check dx block R88-R91 per body area. In examples above id code it as K52, R93.3 and Z85.048.Also keep in mind when provider uses HISTORY of..hopefully he says a date had long time ago or current problem. If in past medical history use Z85,Z86 or Z87 per area and last dx code. But if current cancer add it first per type of Cancer dx code start with C or D letter.I hope this data helps you

Thank you for your help. This is what I needed. I "know" that everyone begins somewhere, but I still feel a bit sheepish because I'll be asking really basic questions. However, one day I will hopefully be like you and able to help newcomers myself.
