Subjective: Patient presents for psychotherapy on smoking cessation. As in previous session, he appears less than committed to stopping.
Patient and provider review patient's reasons for smoking. He says he smokes out mainly out of habit. He has brought his last cigarettes with him. Provider asks if he knows what the warning says on the cigarette packs. He reads the carbon monoxide warning. He says he was unaware that cigarettes contain carbon monoxide. Provider gives him three additional cigarette packs with three different warnings to read. He says he was unaware of most of the information in these warnings. We then review various dangers of smoking. We discuss hypnotherapy in some detail. Provider induces trance. Provider gives patient post-hypnotic suggestions to assist him in becoming a nonsmoker. He reports feeling comfortable and relaxed. He affirms that the relaxation exercise given at the end of the hypnotherapy is helpful with relaxing. Patient agrees to a follow-up visit in 1 week.
Objective: Patient is neatly dressed in casual clothing. He is alert, oriented, pleasant and cooperative. Mood and affect appear to be mildly depressed and anxious. Patient does not appear to be strongly invested in stopping smoking. He may be here mainly because his wife and daughter are pressuring him to stop smoking.
