Hello All,
My colleagues and I had a discussion around how we would code this scenario, we work in an outpatient setting for a dialysis company. I was curious to get other coders perspectives on how they would code. Based on the below HPI excerpt how would you code for Agent Orange?

HPI: Patient has history of ESRD for 2 years due to Agent Orange Exposure.

If you're solely asking about the Agent Orange coding, I would do Z77.098 - Contact with and (suspected) exposure to other hazardous, chiefly nonmedicinal, chemicals.
