Can you post the actual report? We can't code from "procedures performed." If a R/L HC was truly done and aortography then I would say:

93460/93567

Could possibly be G0275 (instead of 93567), even, depending on documentation.

How you bill this

I would be hesitate using 93461 since you did not mention bypass grafts.
