If a physician does not document a specific time frame for the current problem in the HPI but indicates a "history of", can you still count it towards the "duration"?
 
Duration is more specific than "history of"

Duration is much more specific than "history of." We need to know how long the problem has been occurring .... since yesterday, started last week, saw Dr X on Nov 3 ... something along those lines would give me duration.
