I would like other's opinions on number of elements in this HPI


Patient presents to ED for complant of "needing rehab". Patient states he's a meth and narcotic user and feels that he can't take it any more and needs rehab. He admits to some thoughts of taking an overdose, but states that he's got too high a tolerance and can never do it. No active suicidal thoughts. No prior attempts.

Quality= feels that he can't take it any more
Assoc S/S = some thoughts of taking an overdose
Severity = he's got too high a tolerance

psych ROS = No active suicidal thoughts
