I am new at coding oncology. My doctor references in the Hpi, patients estrogen receptor, stage, etc. how can I use this if Hpi is suppose to be from the patient? Patients don't speak in such medical terms.

Most breast cancer patients do know their estrogen receptor status. I would put it as quality since it is a factor that determines treatment and options.
