We have a pt with known CAD whom sees a cardiologist, but is now having
elective surgery for recurrent cancer. How would you code the HPI & ROS for Cardiac Clearance when he is currently having no cardiac symptoms but has HISTORY CAD with CABG, HTN, Ischemic Cardiomyopathy, CHF, Dyslipidema. Any input on where I may be able to find information to help with the CC coding would be greatly appreciated.

V72.81 would be first listed.....reason for surgery in number two spot all cormorbidities that would be a risk are listed as well
