For a commercial insurance, would "55 y/o female presents to the ED with c/o of bilateral hands." Is this acceptable? All other area's are detailed and the visit qualifies a 99285.

I am not sure what it is that you are asking? The statement above to me is a Chief Complaint - not enough elements for an extended HPI - which you need for a 99285. And unless you missed part of the statement, it looks like the patient is there because she has two hands!! What is wrong with her hands?

Definitely not enough HPI elements for that level of code. Time, severity, moderating factors etc need to be noted for it to comply with what's needed.
