CAN the word S'ignificant' discomfort be used for any HPI Elements?

I was thinking severity or quality?
 
You could use it for either element but I would probably put it towards severity.

Severity is best when it is rated 1-10 but you don't always get that in a note. Any word that gives a picture of how bad it is, (ie minimal, significant, extreme) I tend to apply to severity

I would also go for Severity as the description "Significant" carrying weightage to the degree of pain/discomfort.
