I was wondering if anyone had a sample of a Health Risk Assessment form that is required when billing Medicare AWV (annual wellness visits). Our PMD doctors are billing for this and I'm quite sure they aren't doing this. Does anyone know where I can get a copy of this form? Help!!:eek: Thanks dnp686
