I have a dx of 628.9 (infertility) for 58340/74740 which BCBS will not pay for. Is there anything else? I thought 625.8

I hate to tell you, it depends on the patients coverage on infertility. If she doesn't have that coverage it will be her responsibility. We have had that issue in our office. You have to call to verify benefits before the procedure.
