Humana has been denying charges for inappropriate modifiers for PT charges codes like 97110 or 97140 and now also for 98943 (chiropractic extraspinal adjustment). For the PT charge codes, I was able to find that Humana wants modifiers 96 or 97 instead of Medicare's GP modifier. 97 is working to get those charge lines paid. I can't find any information on what they want or don't want for 98943. I used AT (acute condition) and XS (separate structure). It was billed with the spinal adjustment (98940 AT). The only article I can find seems to be saying to use 59 instead of XS. Does anyone here know for sure what they are wanting?

What did you bill on the claim with 98943? Please list modifiers, diagnoses, and other procedure codes. Thanks.
