my doctors have been charging for this a lot and today every dx i used in the
claim was rejected and also V70.0 was rejected as well.
the cpt code 94420 for the health risk assessment test.
i can not get a clear answer as to what is the correct dx to use
for this CPT code.

99420 is your code for the Humana PAF visits.

Was that a typo? If not, that's your problem :)

If it was a typo and you submitted 99420, is your clearinghouse rejecting the claim or the payor? I've seen providers use both V70.0 and problem diagnosis as primary with no problems in payment.
