If a patient had breast cancer with lymph node metastasis, received surgery and chemotherapy, and is no longer receiving active treatment (and it isnt recommended because both the breast and lymph are thought to be "cured") how would the lymph node metastasis be coded at a follow up visit? I understand that the breast cancer would be coded with V-10.3, personal history of malignant neoplasm, breast. However the V10 code tip specifically states to assign the codes in this category only for a personal history of primary malignancy (not metastatic malignancies), and I am wondering how the metastasis is correctly coded?
 
You do not code history of metastasis. The metastatic site in your example is still breast Cancer just now located in the lymph nodes. See the metastatic sites are just an extension of the primary Cancer now located in a different organ.
