Good Morning,
I am hoping someone can help. I have a report that states the indication for the colonsocopy procedure is history of polyps, last colonoscopy 10 years ago. There is nothing actually stating previous polyp/polyps were hyperplastic. Would this colon today be considered an average risk colonsopcy or diagnostic because of hx of polyps. This is a Blue Cross ins.

Is the patient 50 or over ? If they are then you could bill this as screening G0121 w/ V76.51 . If not, some BC plans accept the V76.51 diagnosis as screening even with the G0105 CPT (High Risk Screening).
 
Patient is 63 yrs. old. Because its been 10 years since the last colon, I can use the G0121 even though they found a polyp/polyps from the previous colon? And I dont need to use V12.72?
